New patients
We would like to welcome you to Cotteswold House. We have a special “welcome pack” available. If you would like one, please contact us. You are also welcome to visit the practice to have a look around prior to making an appointment with one of the dentists. A member of staff will be pleased to show you around the practice. A chance to say “hello” to one of the dentists can usually be arranged if you call in advance.
Prior to your first visit.
We will take contact details over the telephone at the time you book your first visit. Other personal details will be recorded when you arrive at the practice. For your own safety, we need to review your medical history. A medical form is available to download. Please complete this as far as possible and bring it to your first appointment. If you have not obtained a welcome pack, please ask for one.
Initial Consultation.
This is an opportunity for you to meet your new dental team and to tell us about any worries or concerns you may have about your teeth. Please feel free to explain things in detail. All information is treated in the strictest of confidence.
Your new dentist will undertake an extensive examination. This will include not only your teeth and gums but also the other parts of your mouth and the rest of your face. Radiographs (X rays) may be indicated to obtain a full picture of any problems you may have.
Following this, your dentist will inform you about any problems they have found. Putting this together with any concerns you may have raised, possible treatments will be described with costs explained and alternatives when available. You will be informed about the pros and cons of each option. You and your dentist will then arrive at an agreed treatment plan. You will be given a full written estimate. Treatment appointments will then be arranged.
For complex cases, your dentist may require time to review all the information and will arrange to see you at a second visit where they will give you the treatment options as above. In these cases, a detailed list of treatment options can be given in writing so that you can consider these in full and have the opportunity to ask further questions before deciding on a definitive treatment plan.
